We replace it under the service — typically with advance-ship for critical devices so you're not waiting on a carrier. Installation and configuration of the replacement is included.

Varies by contract. Typical HaaS agreements include a refresh at end-of-term rather than purchase — the old gear goes back to us and gets recycled, and you get current-generation hardware. Purchase options available at depreciated cost if preferred.
